The deadline for employees to complete Step 1 of the program has been extended until this Friday, November 4.ÌýEmployees may have the Physician Form completed by their provider and submitted to theÌý by the end of the day.Ìý

Employees who completed Step 1 and wish to qualify for the $400 medical premium credit for 2017 via the USNH MyPath2WellnessÌýprogram must complete Step 2, the online health assessment, before the deadline of November 18, 2016.

The MyPath2Wellness plan offered through USNH provides the incentive upon completion of both parts of the program. Receipt of the incentive is not contingent upon achievement of any specific results on either your biometric screening or health assessment.
